If no, then please provide as many specifics about the issue as possible. Is the cat peeing, pooping or doing both outside the box? When did this challenge start? How often does this happen? Where in the home is the cat improperly eliminating?
Tell us about the symptoms. How long has the cat been experiencing the issue? When you took the cat to a veterinarian, what diagnosis and treatment did they offer?
If yes, then please provide details. What were the circumstances of the bite? How severe was the bite? Was skin broken or blood drawn? Have there been multiple incidences of biting? What date(s) did the bite(s) happen?
If you did not obtain the cat as a kitten, then please provide as much information as you are aware of the cat's life before you and how many homes the cat has lived in.
For example, describe if the cat greets them, hides at first, becomes over-stimulated, etc.

Are the children respectful of the cat's space? How does the cat react to the children? Does the cat enjoy the interactions with the children or do they, for example, hide under the bed for the remainder of the children's visit?

What are their names, size, breed, age, altered status (spayed, neutered or intact)? Describe the cats' usual interactions together. For example, do the cats avoid each other at all costs or do they groom each other?

Tell us the cat's preferences. Do they favor scratching vertically or horizontally, and what sort of texture does the cat prefer - carpet, cardboard, rope, letter, etc.? Does the cat have scratching posts, and do they use them or prefer scratching rugs and couches?
Is your companion animal on any monthly topical preventative? These types of preventatives help prevent fleas, ticks, mosquitoes, etc and are in the form of a topical treatment usually placed between the shoulder blades in the fur. Sometimes for cats, it can be in pill form as well.(Required)
What brand of food does the cat eat? Is it wet, dry or both? Does the cat prefer the food room temperature or cold? How much and how often are they fed? Is the cat free-fed or is the cat fed once or twice a day? Where in the house are they fed? If the cat lives in a home with other cats or dogs, where/how is the cat fed in relation to the other animals?
